Cuviva and smartQare forge strategic partnership to transform hospital-to-home care

In response to growing pressure on healthcare systems worldwide, Cuviva AB and smartQare BV today announced a strategic partnership aimed at transforming how patients are monitored and cared for at home, particularly in the critical period following hospital discharge.
By combining smartQare's continuous, wearable monitoring technology with Cuviva's remote care coordination platform, the partners are creating an integrated hospital-to-home care ecosystem. Both solutions are medicalgrade, CE-marked under MDR Class IIa, and clinically validated.
The collaboration will initially focus on chronically ill, elderly, and high-risk patients, empowering clinicians to deliver safe, proactive, and cost-effective care beyond hospital walls.

Immediate Next Steps
The partnership is already in motion, with a phased rollout plan focused on rapid deployment and measurable outcomes. Customer projects are being launched both in and outside Europe, focusing on COPD, cardiac, diabetic, and geriatric care. Joint co-creation projects are underway to validate and refine the integrated solution, with AI-powered predictive alerts to be introduced in later stages to proactively manage patient deterioration.
The joint offering will be available to hospitals, home-care providers, and insurers, with a subscription-based model with reimbursement integration to support scalable adoption.
