smartQare and Cuviva Support Discovery Care at Home Programme in South Africa
smartQare and Cuviva have partnered with Discovery Health, South Africa’s largest medical scheme administrator and managed care provider, to strengthen the technology foundation of the Discovery Care at Home programme through integrated remote patient monitoring and coordinated digital care workflows.
The collaboration combines smartQare’s viQtor medical-grade wearable monitoring technology with Cuviva’s Hospital at Home platform to support safe, scalable hospital-at-home care. Together, the solutions provide clinicians with continuous access to real-time patient monitoring data, structured clinical workflows, patient-reported information and coordinated care pathways — enabling timely intervention and informed clinical decision-making beyond the hospital setting.
Discovery Health supports approximately 2.7 million members and estimates that around 160,000 annual hospital admissions could potentially be managed through Care at Home. The programme enables eligible patients to receive hospital-level care at home while remaining under continuous clinical oversight and remote monitoring.
