smartQare Supports One of Europe’s Largest Hospital-at-Home Programs in Region Stockholm
smartQare is proud to support one of Europe’s most ambitious hospital-at-home initiatives, launched by Region Stockholm and led by Karolinska University Hospital in collaboration with Philips, Cuviva AB, and Vingmed AB. The program marks a major milestone in the future of connected healthcare, enabling hospital-level care to be safely delivered in patients’ homes across the Stockholm region.
As part of the consortium, smartQare will provide its viQtor wearable monitoring solution, enabling continuous remote patient monitoring through real-time clinical insights and integrated connectivity. The initiative is expected to support up to 15,000 patients annually and demonstrates how scalable hospital-at-home care can become a reality through strong collaboration and interoperable healthcare technology.
The program is designed to support patients with complex or long-term chronic health conditions who require close clinical supervision outside the hospital environment. Through continuous monitoring of vital signs and advanced physiological parameters, healthcare professionals can maintain greater visibility into patient status remotely, supporting earlier intervention and more proactive care decisions when needed.
In addition to continuous monitoring, the connected care model enables closer interaction between patients and healthcare professionals through digital care pathways, virtual consultations, and secure communication tools. This creates a more seamless and coordinated care experience while helping clinicians stay connected to patients throughout their recovery or treatment journey at home.
